I have a newborn. How can I get her registered in my passport and should I do that? If I get her registered in my passport will I have to get a visa for her in case of travelling to Ukraine? I remember seen one time a reference to a web site where I can get an application for passport. I am slightly lost what to do in terms of paper work. Thank you in advance for assistance and advice.

Call to the Ukrainian Consulate, and they will prompt you what and how to do.
I would strictly recommend to register your child as an Ukrainian citizen; in such a case you need no visa for her to enter Ukraine.
