A node or nodal mass must be 10 cm or greater to be recorded as "bulky." Alternatively, bulky mediastinal involvement can be defined radiographically, as noted above. Other definitions of bulk disease have been used to define patients with a worse prognosis, such as 6 cm and 5 cm. In the Stanford V protocol, bulky disease, defined as 5 cm or greater required the use of involved field irradiation following completion of chemotherapy.
There is no 1 specific definition. U r right in the fact that having a tumor that is 10 cm makes the job easy, however, that is not the only definition criteria for bulky. Size of the tumor is also not the only thing that makes disease unfavorable. Patients with mediastinal presentation have a worse prognosis than those that did not, another factor for radiation.
From Uptodate:
Unfavorable prognosis — Patients with unfavorable prognosis stage I-II disease receive chemotherapy followed by involved field radiotherapy in a combined modality program. Chemotherapy usually consists of ABVD for four to six monthly cycles in patients with large mediastinal adenopathy or for as few as three to four monthly cycles in patients with other unfavorable prognostic factors.
Unfavorable features include:
-Large mediastinal adenopathy
-Involvement of four or more lymph node regions
-Age over 50 at diagnosis
-A defined combination of B symptoms and elevated erythrocyte sedimentation rate (ESR): B symptoms and an ESR over 30 mm/h or an ESR over 50 mm/h without B symptoms.
*B symptoms are:
-Unexplained weight loss of more than 10 percent of body weight during the six months before initial staging investigation
-Unexplained persistent or recurrent fever with temperatures above 38ºC during the previous month
-Recurrent drenching night sweats during the previous month.
