If there is a strong suspicion for TB and sputum AFB stain and TB cultures are negative, then you have to do bronch.

А может если клинически очень похоже на TB (fever, chills, blood stained sputum, loss of weight) и точно известно, что человек был exposed то TB, лучше начать его лечить без invasive tests?

Anti-TB pills are way too toxic to give them to everybody ;)
But if, for example, there is a cavitary lesion in the upper lobes strongly suspicious for TB, then you might consider starting treatment without confirmation with all 4 drugs (Foley mentioned already) and do further investigation.
