А кто определяет ети errors?

берут архивные записи из госпиталей (admission records, discharge records... как там они называюца) и изучают по этим записям.
причем всплывают явные нарушения, повлекшие смерть, поэтому скорее всего эти цифры занижены поскольку все случаи ошибок невозможно вскрыть чиста изучая архивы...
