In order to file each petition at the U.S. Consulate in Vladivostok, an American citizen must submit in the following order:
All necessary forms you may find at the Forms, Fees and Fingerprints information center:
1) Petition for Alien Relative (form I-130) for each alien relative
2) Four biographic forms (form G-325 A) for the American citizen petitioner
3) Four biographic forms (form G-325 A) for the beneficiary
4) Two photographs each of alien relative (front view, 2 inches by 2 inches, must be in color, taken against a white or off-white background) and the U.S. citizen (regular passport photo) stapled to the lower left corner of the G-325A biographic data form
5) Original and one photocopy of U.S. passport of the petitioner
6) Original and one photocopy of alien relative's birth certificate and English translation
7) Original marriage certificate, photocopy and translation
8) Proof of termination of all previous marriages of the American citizen and alien relative (certified copies of divorce decrees or death certificates of previous spouses)
Please note that translations do not need to be notarized.
The following documents must also be submitted during the visa interview in Moscow:
9) Affidavit of Support (Form I-864). One original, notarized I-864 Affidavit of Support must be submitted for each immigrant visa application from each sponsor, co-sponsor (I-864A), and joint sponsor (I-864).
10) Police report. Immigrant visa applicants must present a police report from every place in which they have lived for over six months since the age of 16. These police reports may be obtained through a local or regional MVD (Ministry of Internal Affairs) branch.
11) Medical examination. The medical examination consists of a physical examination and the administration of all necessary vaccinations. It costs about $100 USD and can be done in a week at the clinic in Moscow or by the U.S. Consulate's panel physician in Vladivostok - Valentina Nikolayevna Luchaninova (work phone (4232) 40-13-35, home phone and anwering machine (4232) 45-24-26), e-mail Dr. Luchaninova.
